Focus Group Participation Agreement
This Focus Group Participation Agreement (“Agreement”) is entered into between Kodiak Creative (“Organizer”) and the undersigned participant (“Participant”).
1. Purpose
Participant agrees to take part in a usability and feedback focus group related to the review of the DuTrac Community Credit Union website prior to its public launch. The purpose of the focus group is to gather feedback to improve clarity, usability, and overall user experience.
2. Scope of Participation
Participant agrees to complete the following activities:
Initial Survey:
Completion of an online survey designed to collect baseline feedback and user impressions.
Pre-Launch Review & Follow-Up Survey:
Review of the website and completion of a second survey or set of questions prior to launch.
Participant understands that participation requires thoughtful, honest feedback and completion of both stages to receive full compensation.
3. Compensation
Participant will receive a total of $150 for full participation, distributed as follows:
$75 upon completion of the initial survey
$75 upon completion of the pre-launch review and second survey
Payments will be issued via Check, PayPal or Venmo within ten business days of completing each survey.
Failure to complete a survey may result in forfeiture of compensation for that phase.
4. Voluntary Participation
Participation in the focus group is voluntary. Participant may withdraw at any time; however, compensation will only be provided for completed portions of the focus group.
5. Confidentiality
Participant agrees that any non-public information, materials, designs, or concepts shared during the focus group are confidential and may not be shared, distributed, or discussed outside of the focus group without written permission from the Organizer.
6. Use of Feedback
Participant acknowledges that all feedback, comments, and suggestions provided may be used by the Organizer for research, design, marketing, or development purposes without additional compensation. Participant waives any ownership or intellectual property claims related to submitted feedback.
7. No Employment Relationship
Participation in the focus group does not create an employment, contractor, or agency relationship between Participant and Organizer.
8. Limitation of Liability
Participant agrees that the Organizer shall not be liable for any indirect or consequential damages arising from participation in the focus group.
9. Governing Law
This Agreement shall be governed by and construed in accordance with the laws of the State of Iowa.
